Upstate Cerebral Palsy is Hiring a Supported Housing Case Manager (level 2) *$1,500 sign-on bonus! Near Utica, NY

Pay $20.00 - $22.00 an hour
The Supportive Housing Case Manager 2 assists program clients in securing and maintaining stable housing which meets required habitability standards. This is done through assessing client needs, referring to additional services, and through the direct service provision of Tenancy Support Services.
Core Responsibilities
  • Assist individuals in all activities related to locating and securing housing.
  • Case Manager will meet with clients face to face at least monthly and will participate in home visits.
  • Develops an individualized plan with the client to ensure the individual remains housed.
  • Schedule and conduct assessments, evaluate needs, and develop treatment planning processes for adults.
  • Develop and maintain an effective communication network with the client, treatment team staff, families, community agencies and other agency departments.
  • Maintain regular contact with client's landlord regarding needs of client or landlord and ensuring lease stipulations are met by both parties.
  • Provide referrals for clients and/or their families to appropriate services. Model to client how to advocate with various community resources available.
  • Provide advocacy, direction and support to clients and/or their families as needed.
  • Complete all required documentation according to program regulations/guidelines.
Qualifications
  • Associates or Bachelors (preferred) Degree in Human Services, Sociology, Psychology or related field.
  • 2 years of case management related experience working with people with mental health and Substance Use diagnosis.
  • Must have a valid NYS Driver's License.
