Registered Nurse (RN) Admission Nurse Job Summary: Great New Pay Scale June 2023

A Registered Nurse (RN) Admission Nurse's primary duties are to review admission documentation to establish care plan goals and perform assessments of newly admitted and re-admitted residents. Discharge Planning with patient and caregiver education for a safe transition home is an essential part of the role. Admission to Discharge, you are an important member of the Interdisiplinary Team. The nurse will provide direct care for all new admissions, review transfer documentation, review and verify physician's orders, and ensure a safe and successful admission process. 

Current Openings: This is a Full Time, Salaried, exempt position. The hours are 2pm-10pm. May be negotiable. Monday through Friday Participation in a weekend on-call rotation required. 

Registered Nurse (RN) Admission Nurse ESSENTIAL FUNCTIONS:

1. Ensures that the highest degree of quality patient care is maintained.

2. Assesses all potential admissions from the hospital prior to transfer to the facility ensuring appropriateness for transfer;

3. Performs a thorough and accurate medication reconciliation for all admissions;

4. Evaluates residents for changes in condition including exacerbation of chronic conditions;

5. Communicates findings with providers prior to hospital transfer;

6. Maintains proper infection control practices;

7. Tracks all admissions and discharges

Registered Nurse (RN) Admission Nurse Requirements/Qualifications:

• Minimum of 1 years of acute care clinical nursing experience with demonstrable skills in clinical assessments.

• Unrestricted license in NYS as a Registered Nurse
