Qualifications:

• Active California Nurse Practitioner license
• 1 year's experience as a Nurse Practitioner
• Experience providing any in-home service, or, a strong comfortability providing healthcare services in a home setting
• At least 6 month's experience providing Medicare Annual Wellness visits and working in Family Medicine or Internal Medicine
• Able and willing to work at least 16 hours per week over two, 8-hour shifts - one of which must be a weekday.

Nice to Haves:

• Fluent in Korean or Spanish

Your Day-to-Day:

• Complete 6 home visits per day throughout Los Angeles County.
