Job Summary

Responsible for optimal patient outcomes throughout the Continuum of Care for specific populations. Scope of responsibility includes interdisciplinary collaboration and management of resources to ensure safe, effective delivery of patient care services. Serves as an integral member of the patient centered medical home team.

Responsibilities

• Conducts initial and ongoing age specific assessments
• Completes medical record documentation 
• Collaborates with other clinical disciplines, involves patients, family, and care partners in the

development of the plan of care 

• Prioritizes nursing actions and intervenes based on changes in the patient’s condition, acuity level,

continuing care needs, and desired outcomes

• Evaluates effectiveness of the plan of care
• Implements physicians orders accurately within appropriate time frames
• Educates patient/care partners/family based on an assessment of learning needs, ability,

and readiness to learn

• Demonstrates continuous customer service competency in every action and interaction

with patients, families, care partners, and co-workers

Years of Experience

• 1 year experience with related populations
• Registered Nurse (RN) - Valid State of New York License - REQUIRED
• Basic Cardiac Life Support (BCLS) – REQUIRED
• Experience with Athena Health EMR

Skills

• Demonstrated ability for Assessment, Evaluation, and Interpretation of Medical Information and Health Care Planning 
• Excellent communication & analytical skills.

RATE OF PAY: $53.40 (NYSNA UNION)- Please note this is a PER DIEM role for vacation and sick coverage only. There is no set schedule.
