Job Summary: The RN's purpose is to provide quality nursing care to our patients, coordinate care with ancillary staff and to communicate effectively with physicians. This position contributes to the goals of the department by being accountable for the delivery and coordination of patient care at a demonstrated level of safe practice and within the scope of practice as defined by the Florida State Board of Nursing and TGH policy. Along with other members of a care team, this role is responsible for patient outcomes that meet the high standards of care provided by TGH. Required Skills: Degree in Nursing - Required. Current Florida RN Licensure - Required. Previous nursing experience in an acute care setting preferred. Duties & Responsibilitie
Act as a clinical and/or emotional resource for clinical team and provide guidance for complex patients to ensure the highest quality of clinical outcome
Perform thorough physical & psycho-social assessments and identify potential barriers for social determination of health, manage patient medication profile and efficiently communicate with the provider lab & test results to ensure a smooth and positive patient experience
May complete triage/in-basket messages, annual wellness visits, care management & coordination of chronic conditions to support the care team and ensure overall continuity of care
Provide patient education, including reviewing a plan of care if protocols are in place, ensuring patient has a thorough understanding of care
Maintains records of medical and nursing treatments and related services executed by nursing staff through appropriate and accurate documentation and utilization of the electronic medical record resulting in compliant and useful documentation.
Identify gaps in care & contribute to improving team's quality metrics to maintain efficient workflow
Maintains overall patient experience and safety, following all emergency clinical policies and protocols, to allow for optimal experience of care
