Position and Schedule
Pharmacy Nurse (RN)
Full Time
Monday - Friday and on call if needed
Essential Duties & Responsibilities
Coordinate and monitor the Skilled Nursing Facility Pharmacy system. Assists the Dir. Of Nursing in the implementation & training of charge nurses, certified medication aides in following policy and procedures in administration of resident/veterans medications.
Supervises the receipt of medications from the VA and other contracted pharmacies. Conducts audits to validate i.e. med availability, timeliness of administration, medication labeling/storage/destruction and security, etc.
Requires interaction with attending physicians N.P.'s on medication orders, reduction of anti-psychotic medications, antibiotic stewardship, etc. Pharmacy Consultant visits, Rx report & DRR's, Costs out medications for pre-admit approval on veteran residents.
Maintains regular and ongoing communication with facility Medical Director ensuring programs and care services meet the needs of patients / residents.
Completes required forms and documents in accordance with company policy and state and/or federal regulations.
Performs other duties as assigned.
Additional Responsibility :
Infection Preventionist Role tracking, trending & analysis.
Tracks and completes the required CDC/CMS weekly NHSN reporting and participates in care planning, QAPI and other resident care meeting.
Qualifications
Graduate of an accredited school of Nursing.
Valid State licensure as an RN
Experience in Medication Administration to Geriatric population
One to three (1-3) years of management, supervisory, or medication experience in long-term care, acute care, or geriatric nursing.
Proficient in the use of personal computer. Use of PCC as a EMR needed
