Job Overview:

Utah Home Health and Hospice is looking for a dynamic, motivated, and compassionate On-Call Registered Nurse to join our patient -centered care team. The primary responsibility of the home health and hospice RN is to provide direct support to the patient and their families while functioning within a medically approved plan of care. The ideal candidate will embody both compassion and expertise in the field of home health and hospice care.

Principle Duties and Responsibilities:

* Work as part of an interdisciplinary team that includes physicians, nurses, social workers, chaplains, massage therapists, certified nurses aides, etc.

• Perform on-call responsibilities as needed by the organization.
• Work with primary care physicians and the hospice medical director.
• Accept and understand the mission of our home health and hospice.
• Ensure that the educational needs of the patient and family are met.
• Review and document patient needs in care plans, progress notes, etc.
• Monitor patient’s condition and assess their needs to provide the best possible care and advice
• Administer medications and change wound dressings
• Provide instant medical care in emergencies
• Schedule: on call 3-4 days a week (5pm-9am) and two weekends out of the month.
• Travel required

Qualifications:

• Must possess current, unencumbered, active license to practice as a Registered Nurse.
• Must have valid care insurance and driver's license.
• Knowledge of and experience in pain and symptom management.
• Well-developed physical assessment skills.
• Must be able to read, write, speak, and understand the English Language.
• Must possess patience, tact and a positive attitude.
• Hospice/Home Health experience preferred.
• Must have valid CPR.

Job Type: Part-time

Pay: $100.00 - $600.00 per day
