Utopia Home Care of Connecticut is actively seeking a Physical Therapist for our Bridgeport, CT service area.
Overview: The Physical Therapist directs, supervises, evaluates, and provides physical therapy services to clients in the home as prescribed by the attending physician. The Physical Therapist functions under the direction of the Supervisor of Clinical Services and cooperates with the primary care nurse in the coordination of care of the client.
Responsibilities:
Comprehensive evaluation of client's level of function and participation in development of the total client care plan.
Identification of client and family needs for other home health services and referral for same when needed.
Participation in case management conferences.
Instruction of client, family and other agency health care personnel in the client's treatment regime when indicated, including proper safe use of equipment and assistive devices, as applicable.
Supervision of homemaker / home health aides when such personnel are participating in the client's regime.
Participates in discharge planning.
Participates in in-service programs.
Prepares clinical and progress notes.
Acts as a consultant to other agency personnel.
Maintains compliance with all agency policies and procedures.
Functional Abilities:
Visual / hearing ability sufficient to comprehend written and verbal communication.
Ability to verbalize so that the average person can understand.
Good physical stamina.
Physical agility to perform required tasks.

Qualifications:
Graduate of a physical therapy school approved by: The American Physical Therapy Association, or The Committee on Allied Health Education and Accreditation of the American Medical Association, or The Council on Medical Education of the American Medical Association in collaboration with the American Physical Therapy Association.
Registered in the State as a licensed Physical Therapist.
Have at least three (3) years of clinical experience in physical therapy which meets the qualifications of a physical therapy supervisor or have a physical therapy supervisor on staff to supervise and be available by telecommunication at all times for therapists with less than three (3) years experience.
