Position SummaryProvides comprehensive substance abuse assessment and recommends disposition according to standardized level of care criteria. Facilitates linkage with other community agencies. Completes admission procedures for substance abuse programs that patients are referred to and assists with coordination of insurance benefits. Performs telephone triage functions for substance abuse assessments. Provides individual and group therapy throughout the continuum of Substance Abuse services. Develops, implements, and documents the treatment planning process to facilitate the transition through the continuum of care. Provides warm handoff for clients being transferred to substance abuse treatment. Participates in continuous quality improvement activities and educational experiences in support of the departmental philosophy and objectives, as well as Health System
Education/TrainingBachelor’s degree in Social Work, Psychology, or related Mental Health field with a CADC required. Masters degree in Social Work, Psychology or related mental health field preferred. At least three years of clinical experience in an established psychiatric setting and/or substance abuse setting.
Licensure/CertificationPreferred State of Illinois licensure for LCPC, LCSW, or LMFT. Obtains nonviolent crisis intervention certification and BLS within 90 days of hire.
Experience/Skills/AbilitiesExcellent interpersonal skills and ability to effectively deal with all levels within the organization and community. Strong verbal and written communication, organizational and critical decision-making skills. Ability to comfortably speak to groups. Successful completion of annual clinical, age specific and job specific competencies and skills validation tools.
