Role: Mental Health Case Manager Reports to: Director of Case Management FLSA Status: Exempt
Salary: $50,000

Our case managers do this and more including ensuring continuity of care, referrals to needed resources, coordination and monitoring of services, discharge planning, housing support, and intensive goal planning! What will you do? Our Mental Health case manager will provide direct hands on, in person support for a variety of individuals while building genuine, therapeutic, and trusting relationships with others while being sensitive to people experiences and trauma. In this role you will complete required paperwork and documentation and provide referrals and case coordination for individuals receiving services, including mental health, medical, financial, social, educational, vocational, housing, and legal services. Duties are preformed under Rule 79 which include documentation, functional assessments, crisis plans, releases, and individual community support plans in schedule and per performance standards. You will interact with the client on a frequent at minimum once a month, or as needs arise. Further responsibility can include development of relationships with county and state staff, family members, employers, and other agencies as we support the person receiving our services. Overtime this position will allow you to grow, and develop your personal skill set and support this growing division for our company.
To be considered for this opportunity you will bring your skills and talent, with a BA or BS in human services or related area; BSW must have a valid and current social work licensure. Further your valid drivers license, and adequate insurance coverage, and access to an automobile.
