Position DescriptionA Registered Nurse who is responsible for the planning, coordinating, providing and documenting care for terminally ill patients from day of admission through day of discharge. Utilizing physicians orders and professional skills, the Hospice RN develops and implements a hospice plan that meets each patients specific needs and is in compliance with accrediting organization standards, federal and state regulations, reimbursement guidelines as well as the organizations policies and procedures.
Duties/Responsibilities
  • Performs physical and psychosocial assessment on all hospice patients.
  • Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
  • Assists in developing a pain management and symptom management plan to keep the patient as comfortable as possible. Reassess pain and notifies the primary care physician when pain is not manageable on the current medications.
  • Develops a written plan of care for each patient.
  • Provides skilled nursing care.
  • Responds to the physical, mental, social and emotional needs of terminally ill patients and their families. Offers ongoing support and palliative care.
  • Assesses patients condition, initiates plan of care, re-evaluates and updates as necessary.
  • Follows up with obtaining lab results and notifying physician of any significant results.
  • Performs patient care responsibilities considering needs specific to the standard of care for patients age and clinical condition.
  • Demonstrates knowledge of medications and their correct administration based on age of the patient and patients clinical condition.
  • Follows the seven (7) medication rights and reduces the potential for medication errors.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Supports and maintains a culture of safety and quality.
  • Obtains all necessary physician orders and collaborates with physician as patients needs dictate.
  • Initiates needed referrals to other disciplines and/or community resources.
  • Supervises Hospice Aides on an, every other week basis.
  • Understands Hospice philosophy and issues of death and dying.
  • Completes patient documentation on time.
  • Attends inservices and staff meetings as needed.
  • Completes assigned training on time.
  • Completes annual education requirements.
  • Maintains patient confidentiality at all times.
  • Performs other duties as assigned.
Required Skills/Abilities
  • Demonstrate knowledge and skills necessary to provide care to the geriatric and adult population.
  • Compassion and empathy for patients.
  • Attention to detail regarding a patients problems to analyze what they need to ease their pain and to prevent further injury.
  • Interpersonal skills to build positive relationships with patients, families and staff.
  • Communication skills to interact clearly with doctors, family members, patients and the entire team of care.
Qualifications, Education And Experience
  • Graduate of an accredited school of nursing.
  • Current Registered Nurse licensure in the state of Utah.
  • Experience as an RN commensurate with one (1) of the following:
    • One (1) year acute care experience within the last two (2) years
    • One (1) year home health care experience within the last two (2) years
    • One (1) year community health or public health experience within the last two (2) years
    • One (1) year hospice experience within the last two (2) years
  • Current CPR certification.
  • Current TB test.
  • A valid drivers license, reliable auto, and current auto insurance.
Physical Requirements
  • Physical stamina to lift, transfer and assisting patients
  • Ability to use muscles to lift, push, pull, or carry heavy objects.
  • Must be able to travel to conduct patient visits.
  • Regularly required to sit, talk, hear, and use fingers to operate a computer and telephone.
