This is an enormously satisfying role in which to contribute to the well-being of seniors in the community. As the primary link between patients and their physicians, you will coordinate all aspects of medical care, including therapy, medication management and monitoring, nutrition counseling, and preventive health care. The successful candidate will help rehabilitate patients to live independently in their homes.

Responsibilities:

• Plan and track the patient's schedule.
• Manage the patient's schedule.
• Evaluate, pace, and document the patient's activities and reactions to treatments and medications.
• Prepare and serve food and beverages to the patient and family in a timely manner.
• Maintain the cleanliness of the home and surrounding areas in a timely manner.
• Maintain a cheerful demeanor in all public interactions with patients, caregivers, and visitors.
• Keep accurate records of appointments, treatments, and medications.
• Demonstrate proper infection control measures when dealing with patients, visitors, families, and co-workers.
