Position Overview: We are seeking a dynamic and dedicated Floating Clinical Liaison/PASRR Nurse to join our team. This position plays a crucial role in assessing the needs of potential residents and ensuring compliance with PASRR (Pre-Admission Screening and Resident Review) regulations. The Floating Clinical Liaison/PASRR Nurse will serve as a liaison between our facilities, hospitals, and the community to facilitate seamless transitions and optimal care coordination.
Key Responsibilities:
  • Conduct comprehensive assessments of potential residents referred from hospitals or the community to determine clinical eligibility and compliance with PASRR regulations.
  • Collaborate with interdisciplinary teams to review medical records, assess resident needs, and formulate care plans that meet individualized healthcare goals.
  • Coordinate admissions and transfers between facilities, ensuring a smooth transition of care and compliance with regulatory guidelines.
  • Serve as a resource to facility staff regarding PASRR requirements, clinical assessments, and documentation standards.
  • Educate and provide guidance to staff on best practices related to PASRR regulations and compliance.
  • Maintain accurate and up-to-date documentation of assessments, care plans, and communication with healthcare providers and regulatory agencies.
  • Participate in interdisciplinary meetings to discuss resident care plans, progress, and discharge planning.
Qualifications:
  • RN, LPN or MSW with current licensure in the state of MA.
  • Minimum of two years of clinical experience in a skilled nursing facility or related healthcare setting.
  • Strong understanding of PASRR regulations and requirements.
  • Excellent assessment skills and ability to make clinical judgments based on resident needs.
  • Effective communication skills with the ability to collaborate across interdisciplinary teams.
  • Detail-oriented with strong organizational skills and the ability to manage multiple priorities.
