Job Overview:
We are seeking a compassionate and skilled Hospice Nurse to join our team. As a Hospice Nurse, you will provide specialized care to patients with life-limiting illnesses and support their families during this difficult time. You will work closely with a multidisciplinary team to ensure the physical, emotional, and spiritual needs of patients are met.

Duties:
- Assess patients' physical and emotional condition and develop individualized care plans
- Administer medications and treatments as prescribed by the physician
- Monitor patients' vital signs and provide pain management
- Collaborate with the interdisciplinary team to coordinate patient care
- Educate patients and their families on disease progression, symptom management, and end-of-life care options
- Assist with discharge planning and ensure smooth transitions for patients
- Maintain accurate and up-to-date patient records

Skills:
- Proficient in specimen collection processing
- Strong nursing skills with the ability to provide comprehensive care
- Experience working with people with developmental disabilities is a plus
- Knowledge of discharge planning systems and processes
- Familiarity with ICD-9 coding for accurate documentation
- Ability to perform procedures using aseptic technique
- Experience with electronic medical record systems, such as eClinicalWorks, is preferred
- Knowledge of botulinum toxin treatment for symptom management
