Veterans Health Administration is Hiring a Physician (Geriatric outpatient clinic - GeriPACT) Near Lexington, KY

The GeriPACT Physician practices in the outpatient setting in the Lexington VA Health Care System. The physician is academically and clinically prepared to perform a broad spectrum of patient care services at the expert level. In addition to providing primary health care, the Physician serves as a consultant to nursing and midlevel medical staff for patients with complex treatment or care needs. Geriatric outpatient clinic to include Falls clinic, Palliative care clinic and Dementia clinic.

Qualifications:


To qualify for this position, you must meet the basic requirements as well as any additional requirements (if applicable) listed in the job announcement. Applicants pending the completion of training or license requirements may be referred and tentatively selected but may not be hired until all requirements are met. Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation.

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.
  • Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
  • Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), b) OR
    [(2) Those approved by the American Osteopathic Association (AOA),OR
    (3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences.
    Residents currently enrolled in ACGME/AOA accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as 'Physician Resident Providers' (PRPs). PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee-basis. PRPs are not considered independent practitioners and will not be privileged; rather, they are to have a 'scope of practice' that allows them to perform certain restricted duties under supervision. Additionally, surgery residents in gap years may also be appointed as PRPs.
  • Proficiency in spoken and written English.

Highly Preferred Experience: Preferred experience with Geriatric patients. Board certified/ board eligible - Internal, Family, or Geriatric Medicine.

Responsibilities:

The Physician works collaboratively with other interdisciplinary team members to provide quality care in a geriatric outpatient clinic setting to include Falls clinic, Palliative care clinic and Dementia clinic.

Duties include, but are not limited to:
  • Patient Care: assures that care is provided in a manner which is in accordance with accepted practices and is evidenced based. Assure that care is provided in a timely manner and in an appropriate setting.
  • As a collaborative Physician, assures appropriate oversight of delegated care activities of Nurse Practitioners and others practicing with a Scope of Practice.
  • Assures timely documentation of clinic encounters in CPRS.
  • Facilitate appropriate consultation and implementation of consultant recommendations.
  • Address issues related to infection control include skin testing, immunization and infection prevention.
  • Optimizes medication regimens by reviewing polypharmacy and potentially inappropriate medications in collaboration with PharmD.
  • Position has potential to develop into an Administrative position as GeriPACT Medical Director.
  • Leadership Responsibilities: organizing staff meeting, communication both down to staff and up to leadership, to include heads up (for acute patient bad outcomes) and issue briefs, acting as Associate Service Chief when the Associate Chief of Staff is on leave. b. Staff Management: routine ratings of program clinical staff, including mid-year evaluations, OPPE's (2x a year), FPPE's as requested by PSB, promotions, disciplinary actions & grievances, training requirements (and TMS capability), employee health requirements, Peer Reviews, ensuring staff productivity.
  • Business Planning: Work with ACOS GEC office on DSS mapping, FTE needs, capacity analysis, workloads, resource needs (equipment, IT, supplies, space, etc.).
  • Health Information Management: Open encounter & open consult management, CPRS documentation requirements, coordinate creation and acceptance of templates.
  • Oversight of relevant performance measures and development of the new focus on Patient Centered Care, Utilization, Quality and Patient Safety oversight, Process Improvement (System Redesign), actively participate in Performance Improvement Committee, provide physician representation during surveys including JC and OIG.
  • Coordinate and facilitating coverage for providers on leave.
  • Establish the Scope of Practice for each non- physician provider, assures that prescribing practices fall with in prescribing authority. On an annual and as needed basis, revises the Scope of Practice and prescribing authority of the non-physician provider on staff on the basis of achieved competencies and performance.
  • Responsible for safety and environmental standards for the GeriPACT and specialty clinics.
  • Other duties as assigned.
