Vida Medical Centers is Hiring a Primary Care Practitioner (APRN) Near Miami, FL

Join our Vida Medical Centers' Clinical team, now with 1 position available at the Miami Lakes location. Company with great benefits including paid time-off, holidays and bonus. Minimum of 2 years' experience as a Nurse Practitioner is required for this position.

Under general supervision of the Medical Director, the Primary Care Practitioner is responsible for providing high quality, comprehensive healthcare services to members of Vida Medical Centers. APRNs provide the full scope of primary care services, including but not limited to diagnosis, treatment, medication management, coordination of care, preventive care and health maintenance of their patients. This position is expected to be familiar with the philosophy, goals and objectives of Vida Medical Centers and work collaboratively to achieve them, as well as comply with the policies and procedures established by the company and pertinent state and federal regulations.

This position operates in a dynamic and professional environment to ensure the highest level of quality healthcare is delivered to our members. Job duties include, but are not limited to:

  • Promote preventive healthcare measures including annual wellness visits, physical examinations, screenings, immunizations, and lifestyle modifications to prevent disease progression and maintain overall health and well-being.
  • Ensure compliance with Healthcare Effectiveness Data and Information Set (HEDIS) measures by implementing evidence-based practices, conducting preventive screenings, and documenting quality metrics related to preventive care, chronic disease management, and patient outcomes.
  • Conduct thorough geriatric assessments to evaluate patients' physical, cognitive, and functional status, and develop personalized care plans tailored to individual needs.
  • Provide comprehensive primary care services to patients, including routine check-ups, diagnosis, treatment, and management of acute and chronic conditions.
  • Prescribe appropriate medications, monitor medication adherence, conduct medication reconciliations, and adjust treatment regimens as necessary to minimize polypharmacy and prevent adverse drug reactions.
  • Maintain accurate and up-to-date medical records, documentation, and progress notes in compliance with regulatory standards and company organizational policies.
  • Educate patients and their families about health conditions, treatment options, medication management, and strategies for optimizing health and independence in older age.
  • Assist walk-in patients, addressing their urgent concerns and following up every 24/48/72 hours as required. Medical emergencies should be addressed immediately with appropriate intervention and/or by contacting 911.
  • Daily review of incoming clinical reports and results (e.g. labs, radiology reports, specialist medical notes) sign, date, and follow-up accordingly in a timely manner.
  • Order and document medical referrals for patients requiring services in-house and services not offered at Vida Medical Centers. To guarantee more efficient clinical communication with other peers, use every effort to persuade patients to attend one of our in-house specialists when available.
  • Continuously monitor patient flow, wait times, and schedules, along with the Site Manager to optimize patient comfort and office efficiency.
  • Collaborate with interdisciplinary healthcare team members including nurses, medical assistants, nurse practitioners, social workers, and specialists to coordinate care, facilitate referrals, and ensure continuity of care for patients.
  • Work with Administrative teams such as Medical Records, Referrals, Surgical Clearance, Hospitalization, Customer Service, Retention, etc. to ensure a cohesive, accurate, complete, and efficient patient experience.
  • Accurately assign and document appropriate medical diagnosis and CPT codes for services rendered during patient encounters, ensuring compliance with coding guidelines and regulations.
  • Review monthly reports to ensure target metrics are achieved in a timely manner
  • Using company software and reporting system, manage medical utilization by evaluating the appropriateness and cost-effectiveness of diagnostic tests, procedures, referrals, prescriptions, and treatment modalities, while ensuring high-quality care and patient satisfaction.
  • Regularly monitor and answer phone calls, faxes, EMR messages, and emails in a timely manner.
  • Ensure strict confidentiality of all health records and member information, adhering to relevant healthcare regulations, including HIPAA guidelines.
  • Support and uphold the values, policies, and procedures of Vida Medical Centers. Follow established departmental and medical center policies, procedures, and objectives.
  • Provide extraordinary customer service to all members and internal and external staff, including specialist offices.
  • Ensure maximum patient satisfaction and exceptional quality care among all members to optimize retention rates. Develop and carry out strategies to maintain a high retention rate within your member panel.
  • Stay abreast of latest advancements in geriatric medicine, evidence-based practices, and clinical guidelines through ongoing education, training, and participation in professional development activities.
  • Follow continuous quality improvement objectives and safety, environmental, and/or infection control standards.
  • Track and maintain updated professional licenses in compliance with regulatory agencies and participant Healthplans and provide copies to the Vida Medical Centers administration in a timely manner.
  • Participate in trainings and staff meetings as required.
  • Maintain a clean and safe work environment.
  • Comply with Vida Medical Centers on-call afterhours service rotation requirements.
  • Perform other duties as assigned by supervisor.

Skills Required:
