View Point Health Job Title : Residential Care Provider (RCP)Job Code : SSS010 / Client Support WorkerShift : 2nd or 3rd ShiftFull-Time(40 hours per week on average)Job Hours : 3 : 00pm - 11 : 00pm or 11 : 00pm - 7 : 00amBase Location : Delta ITR - Covington, GADivision / Department / Program : Adult MH Supportive Housing Intensive Under direct supervision, assists with daily living activities at the person's home, in a daytime non-residential facility or a residential facility.

Provides training of basic personal and social skills, simple academics, or work skills to consumers / students in a state school, hospital, juvenile detention facility, residence, group home, workshop, or supported employment environment.

The Residential Care Provider monitors general resident activity to observe and report behavior / progress of individuals with severe developmental disabilities living in a group home or other community residential setting and provides individualized skills trainings according to residents needs.

Duties & Responsibilities : Determines the individuals strengths, needs, abilities, and preferences by observingdaily activities and reviewing records.

Assists individuals in basic problem-solving and daily decision-making using the least intrusive methods appropriate for each.

Provides training and oversight in daily living skills and personal care, such as dressing, hygiene, housekeeping, cooking, laundry, money management, and socialization skills using techniques appropriate for their abilities.

Maintains the residence in a clean and organized condition.Plans, prepares, and serves balanced meals.Transports individuals to work, day programs, doctor appointments, shopping, recreational and other activities.

Supervises the self-administration of individuals medications using agency protocols. Maintains required records.Documents individuals progress toward goals and objectives identified in their treatment and / or behavior plans through activity / progress notes, data-tracking sheets, and / or other prescribed skill builder formats.

Completes all home and vehicle records, logs, notes, assessments, reports, and other documentation according to protocols.

Lifts and carries groceries and supplies and moves furniture for cleaning. Assists disabled individuals on / off vehicle and secures seatbelts.

Performs other duties that require moderate lifting. May use a gait belt or mechanical lift to lift and position individuals, as needed.

Minimum Qualifications : High school diploma or GED AND any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities.

Must be 21 years of age, have valid Georgia Driver's License, and a clean motor vehicle report. Note : Some positions may require a valid driver's license.

Preferred Qualifications : Experience working with MH / IDD population.Certified Nursing Assistant certification (CNA), Direct Support Professional (DSP), Direct Care Professional (DCP) or Patient Care Technician (PCT) preferred.

Requirements / Competencies : Must pass a pre-employment drug screen and subject to random drug screensMust have a valid Drivers License and a clean Motor Vehicle Record in accordance with company policyMust pass a criminal background check (including fingerprinting)Requires TB Evaluation at pre-employment and annuallyRequires negative COVID test at employmentMust be able to lift 20 poundsRequires long periods of sitting / standingMust have the following technical skills Word, Excel and Electronic Health Record (Care Logic)Must be willing to transport residents in company vanMust demonstrate the ability to maintain professional boundaries with clients in a residential treatment settingMust be able to lift and carry groceries and supplies and move furniture for cleaning.
