View Point Health's Centerville Play Therapy & Resilience Training Center offers culturally, linguistically, and developmentally appropriate education & practice of resilience skills through play and family therapy focusing on infants through age 17 and their family. Case Manager/Community Support works closely with children and their family to identify needs related to social determinants of health to maximize continuity of care through direct support and skills training, referral and service linkages, continued care monitoring, and collaboration with the program’s interdisciplinary team and other external partners. Duties are performed in a fast-paced environment that requires abilities to demonstrate organization skills, professionalism, solution seeking skills, and motivation to serve the community.

Under broad supervision, instructs or trains clients and/or students in a specific area of assignment as part of a program that includes an organized agenda of practical and workforce readiness instruction. May serve as lead worker.

Duties & Responsibilities:

  • Responsible for case management activities working directly with children, adolescents, caregivers, family members, school staff, community stakeholders and partners through in-home, school or community visits which includes community support (CSI billable activities with contact requirements) and assistance in the development of interpersonal, community coping and functional skills (related to stressors at home, school, social environments)
  • Provides strength-based interventions and focuses on developing and practicing resilience skills that are directly associated to individual needs and related treatment plan. Assesses and addresses school related issues: behavior, attendance, absenteeism, and barriers.
  • Uses social determinants of health and other screening/monitoring tools to identify client and family needs. Collects program specific information using the electronic health record to document activities and advocacy efforts within 24 hours required entry
  • Participates in program or agency activities and actively maintains engagement with parent, school and community provider and outreach events to promote behavioral health services
  • Collaborates with interdisciplinary team to manage resource and referral connections within community and to reduce behavioral health disparities
  • Provides assistance with launching and closing technology for telehealth visits as needed
 

Minimum Qualifications:

Bachelor's degree in a related field from an accredited college or university OR associate’s degree from an accredited college or university OR Vocational/Technical degree in area of assignment from an accredited college AND Two years of experience in the area of assignment OR One year of experience required at the lower-level Practical Instructor 2 (EDT011) or position equivalent.

Preferred Qualifications:

  • Superior verbal and written communication skills
  • Experience with CareLogic or other ECR/EMR
  • At least 2 years’ experience working in behavioral health care OR child and family serving organization
  • Bilingual - ability to read, write, and speak in Spanish and English

Requirements/Competencies:

  • Ability to multi-task and manage time.
  • Must be able to lift 20 pounds.
  • Requires long periods of sitting/standing.
  • Must have valid Georgia driver’s license and Motor Vehicle Record in accordance with company policy.
  • Candidates for selection must pass a criminal background check (including fingerprinting)
  • Must pass a pre-employment drug screen and subject to random drug screens.
  • Demonstrates sensitivity and expertise in diverse cultural and linguistic needs.
  • Demonstrates commitment to the agency’s mission, vision, and values.
  • Other duties as assigned.
