Chief Purpose of this position: The Department of Internal Medicine, Division of Geriatric Medicine is seeking an incumbent to work within the Advanced Geriatric Care Program.The Advanced Geriatric Care Program isa multi-faceted care system including: office-based medical care for those whoare comparatively mobile; home-based care both short-term and long-term forthose that are less mobile; nursing home care; inpatient consult service;behavioral health counseling; pharmacy consultation; fully developed telemedicineservices (triage, counseling, monitoring, video-visits); and care managementand social work services.Patients willreceive primary care if they need or want a new medical home and consultativecare if their medical home is strong but needs augmentation with geriatricconsultation, home care, telemedicine or other consultative service.
Position Responsibilities:
Teaching: To support teaching directly via interactions with fellows, residents, medical students, and non-physician professionals.
Research: To support the research mission of the Division of Geriatric Medicine.
Service: To provide service of the highest quality to the people of the state of Virginia and beyond.
Clinical:To be responsible for managing a panel ofpatients throughout the continuum of care.To assure that care is timely, appropriate, of high quality, and costeffective.To create and enhance systems to assure thatall patients receive planned, evidence-based care and care is fully coordinatedwith ongoing continuity for the patient, family and caregiver.To workcollaboratively with attendings, fellows, residents, NPs, and other staff toensure a team-based approach to care is fostered.To understand and welcome the importance ofroles as care providers, team members, and major influencers of our community'sperception of the geriatrician's quality and service.Sharing night call (onein nine currently).
Other: (administrative duties, etc.) Submit billing and document care as required by the organization.Abide by all medical staff requirements.Participate in quality initiatives.
Required Qualifications:

• Required: BC/BE in Internal Medicine
• Must be comfortable working with aninterdisciplinary team including nurse practitioners and pharmacists, andmust integrate well with members of an extended care team
• Must have direct experience managingcomplex inpatient and outpatient geriatric patients with a solidunderstanding of the geriatrician role and function
• Demonstratedexperience working in and fostering a diverse faculty, staff, and studentenvironment or commitment to do so as a faculty member at VCU

Preferred Qualifications:

• Geriatric fellowship training strongly preferred
• Experience with EPIC electronic health record preferred
