Job Duties
Central State Hospital (CSH) is a 277 bed Joint Commission accredited psychiatric inpatient facility located in Petersburg, Virginia. CSH has proudly been serving citizens of the Commonwealth since 1870, when we opened our doors as the first psychiatric hospital in the country dedicated to the treatment of African Americans. A new and modern CSH is currently under construction on the same property, with a scheduled completion date of 2027.
CSH maintains the only maximum-security treatment facility in the state (111 beds). We also have 166 beds available for individuals in the community who need inpatient care, as well as individuals in the criminal justice system. We believe that treatment should be holistic and focused on the person's individual needs, recognizing that recovery is not linear or the same for every person. Treatment modalities may include medication, evidence-based group and individual therapy, peer support, psychosocial rehabilitation, vocational training, preparation for court, and treatment for substance use.
Job Duties:
- Daily review of Revenue Cycle and Discern Reporting Portal for schedule of patient external medical appointments;
- Export and organize pertinent health information from the patient electronic health record (EHR);
- Provide patient medical appointment packet to nursing to accompany patient on medical appointment;
- Follow-up with consultants to obtain results and/or recommendations for patient care;
- Provide results/recommendations to scanners for scanning information into the patient EHR;
- Monitor and respond to Message Center communications;
- Receive information for new admission, ensure correct patient, label information accordingly;
- Create Patient record/ encounter in Avatar Database entering demographics and CSB information;
- Notify Admission Clinicians of patient entry into the database enabling clinicians to start documentation;
- Scan all records received on admission (preadmission screening, legal, past medical records) into patient EHR under correct folder and naming convention;
- Assists the ROI Officer exporting patient information to fulfill authorized requests;
- Perform admission audits on 100% of admissions to the facility, analyze EHR to determine documentation needed for compliance with facility policies within 7 days of admission;
- Communicate to the clinicians of needed/missing documentation for compliance;
- Maintain spreadsheet of audit results updating throughout the month
Minimum Qualifications
- Knowledgeable in using an electronic health record application for auditing and importing/exporting data.
- Experience using Microsoft Word, Excel and Outlook.
- Experience working as an HIM Specialist/Medical Record Clerk
- Ability to understand and apply HIPPA regulations
