VIRGINIA HEALTH SERVICES is Hiring a Resident Care Coordinator (LPN) - The Hamilton Near Yorktown, VA

The Resident Care Coordinator, LPN reports to the Director of Nursing and is responsible for administering medications and assessing/evaluating assigned residents. This person is the Staff Member in Charge when working.

Essential Duties and Responsibilities:

Complete the resident ISP’s and UAI’s
Performs medication pass in an efficient manner and in accordance with the medical plan of care.
Completes review of Physician Order Sheet with MAR/TAR for accuracy.
Reviews medication administration documentation for completeness and accuracy.
Maintains medication carts in neat and organized manner.
Performs treatments and lab test as designated.
Monitors and maintains systems as assigned by the Director of Nursing/designee.
Must stay awake at all times while on duty.
Must be capable of protecting the physical and mental wellbeing of the residents.
Must be able to carry out the necessary policies and procedures in the case of emergencies.
Assumes duties of the charge nurse in their absence.
Assumes duties at any Virginia Health Services unit/facility as required.
Promotes and demonstrates positive customer service towards VHS, co-workers, residents and visitors.
Ensures that the health care service needs of each resident are met
Performs other duties as assigned.

Qualifications and Requirements:

Graduate of an accredited School of Nursing and holds current Virginia license.
Experience/education in Gerontological Nursing preferred.
Computer knowledge preferred.
Ability to read, write, analyze, and interpret information.
While performing the essential functions of this job, the employee is regularly required to walk, turn, bend, stoop, lift, push, pull, reach, talk, hear, taste and smell.
Must be able to lift, push, pull and move up to 50 pounds.
Risk of exposure to blood borne pathogens and hazardous materials
