Responsibilities
- Perform initial assessment or admission to agency to determine treatment needs of patient within home environment on physical referral only
- Develop a plan of care with measurable goals and specific discipline related treatments in conjunction with physician referral/orders
- Provide direct speech therapy services as outlined in the plan of care including performance of speech therapy procedures, instruction of patient and or caregiver, recommendations for equipment or adaptation of home environment
- Re-evaluate and revise plan of care as needed with physician approval
- Perform scheduling and record keeping procedures in accordance with agency policy
- Perform other related duties as assigned Schedule and Shift Details
- 8a-4:30p, 1 weekend per month and rotation of all national holidays
- Master's degree or higher
- 2 years of experience in ALL of the following:
- Direct Care
- Home Health
- ALL of the following valid licenses/certifications:
- Speech Language Pathology in Ohio (OH)
- American Heart Association Basic Life Support (AHA BLS)
