Responsibilities
Assessment and Evaluation: Conduct comprehensive assessments of residents' functional abilities, cognitive skills, and daily living needs.
Personalized Treatment Plans: Develop individualized care plans tailored to each resident's needs and rehabilitation goals, utilizing evidence-based practices and therapeutic techniques.
Therapeutic Interventions: Implement various interventions, including activities of daily living (ADL) retraining, adaptive equipment training, cognitive rehabilitation, and fall prevention strategies.
Collaboration: Work effectively with the interdisciplinary team, including physicians, nurses, physical therapists, and social workers, to ensure coordinated and holistic care.
Documentation: Maintain accurate and timely documentation of resident assessments, treatment plans, progress notes, and discharge summaries.
Education and Training: Educate residents and families on strategies to enhance independence, safety, and overall well-being.
Qualifications
Education: Master's or Doctoral degree in Occupational Therapy from an accredited program.
Licensure: Valid state license as an Occupational Therapist in Wisconsin.
Experience: Experience in a skilled nursing or geriatric rehabilitation setting is preferred, but new graduates are welcome to apply.
Skills:
Excellent communication and interpersonal skills to build rapport with residents, families, and staff.
Strong clinical skills and knowledge of evidence-based practice in geriatric rehabilitation.
