Responsibilities
Assessment and Evaluation: Conduct comprehensive assessments of residents' communication and swallowing abilities, identifying areas for improvement.
Personalized Treatment Plans: Develop individualized care plans tailored to each resident's needs and rehabilitation goals.
Therapeutic Interventions: Implement various interventions, including speech therapy, language therapy, and swallowing therapy, to improve residents' communication, cognitive, and swallowing skills.
Collaboration: Work effectively with the interdisciplinary team, including physicians, nurses, occupational therapists, and social workers, to ensure coordinated and holistic care.
Progress Monitoring: Continuously assess and document residents' progress, adjusting treatment plans as needed to optimize outcomes.
Discharge Planning: Collaborate with the care team to develop safe and effective discharge plans for residents returning home or transitioning to other care settings.
Education and Training: Educate residents and families on communication strategies, swallowing techniques, and home programs to promote continued progress.
Documentation: Maintain accurate and timely documentation of all evaluations, treatments, and progress notes following facility and regulatory standards.
Qualifications
Education: Master's degree in Speech-Language Pathology from an accredited program.
Licensure: Valid state license as a Speech-Language Pathologist in Washington.
Experience: Experience in a skilled nursing or geriatric rehabilitation setting is preferred, but new graduates are welcome to apply.
Skills:
Excellent communication and interpersonal skills to build rapport with residents, families, and staff.
Strong clinical skills and knowledge of evidence-based practice in geriatric rehabilitation.
