Job Summary:
We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will work in a hospital setting, providing comprehensive patient care and coordinating services to ensure optimal outcomes.
Responsibilities:
- Conduct diagnostic evaluations and assessments for patients
- Collaborate with healthcare professionals to develop and implement patient care plans
- Maintain accurate and up-to-date medical records
- Provide direct patient care in various settings such as medical-surgical units, hospital medicine, and pediatrics
- Coordinate patient discharge planning and post-discharge follow-up care
- Participate in multidisciplinary rounds and case conferences
Requirements:
- Valid RN license or BSN degree
- Valid Drivers License
- Strong clinical skills including patient assessment and care coordination
