Position: Client Care Manager

Location: Office in Aurora and/or Wheat Ridge. Client visits are throughout Aurora and East Denver.

Key Responsibilities:

Client-Centric Focus: Build and maintain warm, empathetic communication channels with clients, their families, and caregivers.

Assessment & Planning: Conduct thorough in-home consultations and manage the process to bring new clients on board. You will also create personalized care plans that champion our client’s needs and well-being.

Collaborative Teamwork: Work seamlessly with our team, ensuring the right caregiver-client matches and facilitating smooth introductions.

Continuous Improvement: Stay proactive in providing ongoing education and resource recommendations to clients and their families, ensuring they feel supported at every stage.

Networking: Build and maintain relationships with select local referral sources (e.g. hospitals, assisted living).

Educational Background: A Bachelor’s degree is preferred. Experience in home care, social work, or a related field is a plus, but not mandatory.
