Visiting Nurse Association Health Group is Hiring a Community Health & Outreach Specialist Near Monmouth, NJ

The Community Health & Outreach Specialist is responsible for community outreach to targeted communities in order to link individuals to the VNACJ Community Health Center for primary care and other pertinent services as well as assisting patients in activities related to their healthcare. The Community Health & Outreach Specialist will also participate in community coalitions, relationship building, and outreach events. In addition to their outreach and patient assistance responsibilities, team members will be cross-trained in the front desk, call center, and patient navigator functions in order to provide backup as needed.
The successful candidate will be entrepreneurial, adaptable, and demonstrate a team-oriented mindset in order to represent the VNACJ Community Health Center’s four Monmouth County locations in new and creative ways within the community.
  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationships with co-workers, patients and members of the community
  • Maintains expertise in eligibility, enrollment and program specifications for health insurance and other community resources;
  • Participates and assists in the planning of community coalitions, relationship building, and outreach events such as health fairs, neighborhood community activities, screenings, and special events;
  • Works with individuals and organizations that provide patient services or resources (i.e. transportation services, counselors, language translators, and representatives who assist with financial support);
  • Enrolls patients in the various Children and Family Health Institute programs and makes referrals to community-based programs;
  • Provides direct community outreach to targeted populations and refers individuals to the VNACJ CHC for primary medical care and dental care
  • Assists in the development and implementation of marketing campaigns to the communities served by VNACJ CHC;
  • Maintains comprehensive documentation for data collection and provides statistical data and/or reports per grant requirements;
  • Promotes a safe and efficient working environment by adhering to VNACJ CHC policies and procedures including those related to absenteeism and appearance;
  • Omission of specific duties does not exclude them from this position if the work is similar, related, or a logical assignment for this position.
  • High School diploma/GED, Associates or Bachelor Degree in health education or social work preferred
  • Bilingual – English/Spanish, preferred
  • 2-3 years of community outreach experience
  • Strong computer skills
  • Excellent communication, customer service, and interpersonal skills
  • Knowledge of health insurance programs, coverage, and payment plans including Medicare, managed Medicaid programs, and Market Place Exchange preferred
  • Knowledge of community health centers, health and human services as well as immigration-related services preferred
  • Complete NJ Department of Health Community Health Worker Training or equivalent in first 6 months of employment.
  • Perform under the pressure of patients and health center staff in order to meet deadlines.
  • Current and valid driver’s license, auto insurance, and registered vehicle.
  • Exposure to weather and temperature extremes.
Working Conditions/Physical Demand Working indoors with telephone, copier, fax machine, computer, printer, centrifuge, electric exam table, glucometer, nebulizer, needles/syringes, oxygen equipment, personal automobile, scales, sphygmomanometer, stethoscope, thermometer, venipuncture equipment, wheelchair.
