- Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
- Attends weekly team meetings, case conferences and clinical supervision meetings
- Assesses and evaluates needs of women and their families as they relate to health and life course development
- Carries a caseload of up to 25 clients and completes home visitation in accordance with the NFP model elements and NFP Visit-to-Visit guidelines
- Provides counseling and instruction to assist women and their families to attain targeted goals
- Develops and maintains community relationships to support client referrals
- Precepts and mentors new hires and makes visits with student nurses as needed
- Adheres to agency infection control and safety policies
- Adheres to the organization’s policy in regards to absenteeism and appearance
- Bachelor’s degree in Nursing
- 2-3 years work related experience
- Strong computer skills
- Excellent verbal and written communication skills
- Excellent interpersonal and organizational skills
- Bilingual- Spanish speaking a plus
