As a Therapist at Vivia Health, you will have a profound impact on improving the lives of patients by providing psychosocial support and treatment. You will collaborate cross-functionally with an interdisciplinary team to design comprehensive and holistic treatment plans covering therapy, medical healthcare, psychiatry, and case management.

What will your role entail?

• Collaborate with other professionals to evaluate patients' medical or physical conditions to assess client needs
• investigate abuse or neglect cases and take authorized protective action when necessary
• Counsel clients and patients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life
• Monitor, evaluate, and document client progress according to measurable goals described in treatment and care plan; modify treatment plans to comply with changes in patients' status
• Identify environmental factors impacting patient progress through interviews and reviewing medical records
• Plan and conduct programs to combat social problems, prevent substance abuse, or improve community health and counselling services

This role is based in our Gage Park clinic. Work hours are dependent upon the caseload.

What do you bring as a therapist?

• 2-5 years of experience
• LSW or LPC certification
• We are open to considering candidates that are not currently licensed that are studying to become licensed or need to complete clinical hours
