Works as a member of an interdisciplinary care team at a NORC site location and is responsible for the integration of health focused information, education, assessment, advocacy, and linkage to care for residents who live within the boundaries of the NORC program. Provides interventions designed to engage residents in self-care management health strategies that support successful community living. Works under general direction.
Compensation:$40.95 - $51.19 Hourly
What You Will Do
License and current registration to practice as a Registered Professional Nurse in New York State required
CPR/BLS Certification required
Associate's Degree in Nursing from an approved program required
Bachelor's Degree in Nursing from an approved program preferred
Minimum of two year of experience working as an RN in LHCSA or CHHA required
Prior experience working with an aging population and providing clinical assessments of their health needs required
Per Diem
