This position is responsible for providing supervision of assigned staff, ensuring that contracted services are being provided to persons served and performing related duties as assigned.

The supervisor reports to the Assistant Director of Northshore Services and is responsible for providing supervision of assigned staff, care coordinator services to persons served, ensuring that quality outcomes are achieved;

preparing documentation; the identification, development and coordination of housing resources and supportive services for individuals who are homeless and disabled, and performing related duties as assigned.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Provides supervision of assigned
• Develops and maintains effective person served services in compliance with VOASELA, Inc. and funding sources.
• Develops, coordinates, and implements service plans and assessments for meeting the housing needs of individuals who are homeless / disabled.
• Coordinates support services and housing placements for persons served who are homeless and

KNOWLEDGE, SKILLS AND ABILITIES

This position requires the incumbent to have completed education at the BA / BS level and three year experience working with individuals who have a disabling condition in a home and community setting.

Requires a demonstrated ability to supervise staff and provide quality outcomes. Position requires employee to have excellent verbal and written communication skills and the ability to function as part of a team.
