The Opportunity: Case Manager with VOAMASS's Housing Outreach to Placement Effort (HOPE)

Job Type: Full-time, On Site 

Pay: 50k/ year

Veteran Services||VOAMASS has assisted over 2,400 veterans in Southeastern Massachusetts to rapidly find both transitional and permanent housing. The success of our veterans’ programs is driven by our dedicated staff with lived experience, and our partners such as the Department of Veterans Affairs, local town Veteran Service Officers, MassHire, several community providers of both civilian and veteran programs, and grassroots veteran organizations.

What you’ll do as Case Manager:

As Case Manager, you’ll have a dynamic and critical role in our HOPE program providing high-intensity veteran outreach and case management to identify and assist homeless veterans and their families to obtain housing. Our programs help people reach their goals and achieve wellness through a Housing First, client-centered, trauma-informed framework. The scope of services will include: direct street outreach, housing counseling and case management, mental health and substance use disorder support, and homeless veteran flexible assistance. As Case Manager you will also develop housing plans and budgets with the household, conduct assessments, make referrals, and coordinate services with VA Medical Centers and other community agencies. 

Case Manager Key Responsibilities:

  • Maintain a caseload of Veteran clients and perform outreach, engagement and advocacy in surrounding service areas.
  • Complete schedules, logs and reports on outreach, including data on referral, intake, assessment and disposition/assignment.
  • Complete all pre-screen, threshold, and enrollment forms and secure related eligibility documents for veteran clients
  • Develop housing plans and budgets with the household and assist in connecting Veterans to resources in order to maintain or obtain permanent housing.
  • Assist with increasing veteran household income and prepare the Veteran family to be able to independently maintain housing and become financially independent alongside our VOAMASS integrated services team.
  • Conduct assessments and coordinate services/referrals with VA Medical Centers and other community agencies.
  • Assist household in applying for housing and subsidy applications, Veterans Administration, Social Security, and other state/federal benefits.
  • Build and maintain relationships with community organizations, partners, and stakeholders. Act as a liaison to foster collaboration and support. 
  • Help identify and remove barriers to permanent housing and complete housing inspections for prospect units.
  • Organize and coordinate events, workshops, and informational sessions to increase awareness and participation and provide education on basic tenant rights & responsibilities.
  • Participate in individual supervision, team meetings, internal and external program audits, trainings, and other professional development.
  • Follow professional standards and ensure that all services are documented and maintenance of persons served records is completed.

The Case Manager role is about making a difference in the lives of the people we serve. As Program Manager, you’ll bring the following qualifications:

  • BA/AA degree preferred; however, a minimum of 2 years related experience in a case management role may be substituted.
  • Knowledge of veteran issues, homelessness, and state & Federal resources highly preferred.
  • Computer proficiency is preferred in electronic health records, Microsoft Word and Outlook.
