Job Summary:

VRD Group Corp is seeking a detail-oriented Intake Coordinator to join our team. The Intake Coordinator will be responsible for gathering medical paperwork and working with the participant/informal supports and other waiver service providers to determine a safe plan of care to be enrolled into the program.

Duties:

- Communicate with customers to understand their service needs
- Document service requests accurately and thoroughly
- Coordinate service appointments and ensure timely completion
- Research technical information related to service requests
- Edit and proofread service documents for accuracy
- Utilize word processing software to create and update service reports
- Collaborate with technicians to ensure clear communication of service requirements -Interact with applicants, providers and informal supports

-Create a throughout detailed service plan for individuals within the intake process within 45 days and submit for approval.

-Assist with first in person visit upon enrollment to transfer to the assigned ongoing Service Coordinator.

Requirements:

- Prior experience working in the NHTD/TBI Waivers
- Strong editing and proofreading skills to ensure accurate documentation
- Excellent communication skills to interact effectively with participants, informal supports, service providers, and team members
- Ability to gather and utilize medical information to create a safe plan of care
- Proficiency in word processing software for creating detailed service reports - Must have knowledge of and experience working with the NHTD/TBI Waiver Programs

This position offers the opportunity to work in a dynamic environment where attention to detail and effective communication are key components of success. This position allows you to primarily work from home with limited home visits of participants and applicants.

Job Types: Full-time, Part-time

Pay: $19.00 - $22.00 per hour

Expected hours: 40 per week
