Job Description:

As a Field Nurse, you will be responsible for conducting face-to-face assessments of members in their homes and/or facilities using NYS UAS Assessment Tools. Your role will involve evaluating functional status, medical, behavioral, psychological, and community resource needs to coordinate, integrate, and monitor medical and health care services.

Essential Responsibilities:

• Conduct face-to-face assessments to determine various needs and levels of work capacity.
• Collaborate with internal and external teams to develop Person-Centered Service Plans.
• Identify and refer appropriate cases to relevant governmental agencies.
• Communicate effectively with stakeholders to ensure coordinated care.
• Attend Fair Hearings with the Medical Director as needed.
• Manage complex assignments and act as a positive role model.
• Ensure adherence to all regulatory and accreditation requirements.
• Assist with process improvements, documentation, and mentoring.

Minimum Qualifications:

• RN with valid NYS Nursing Licensure.
• Valid NYS driver’s license.
• Minimum of four years clinical experience in ambulatory care, community health, or home health.
• Preferred experience in LTSS or MLTC UAS field.
• Excellent communication, computer, and interpersonal skills.
• Ability to handle complex assignments and travel as required.

Physical Requirements:

• Ability to travel across regions.
