POSITION SUMMARY:
The Care Coordinator is clinically responsible for the coordination of new and established patients within the Clinics. This position will work in partnership and collaboratively with all members of the Health Care Team to assure timely access to services, resources and provide continuity of care. The Care Coordinator helps to ensure the patient understands every aspect of their care. This position is expected to work within the program directives set by funding sources within the philosophical framework set by the Board of Directors.
EDUCATION/EXPERIENCE:
1. High School Diploma required
2. Associates/Bachelor’s degree in nursing from an accredited registered nursing program and licensed as a Registered Nurse in the State of Hawaii
3. 5 years clinical experience in an outpatient clinic preferred
4. 2 years of leadership experience preferred
5. Proficient in the following computer skills: word processing, entering data onto and sorting data in spreadsheets, navigating through the internet, email, and experience with electronic health records
6. Communication skills (verbal and written) required
7. Ability to work in partnership with community health organizations
8. Ability to communicate effectively with staff members, community members, and patients
9. OSHA training annually
10. BCLS every two years
