Walker Healthforce is seeking a RN Case Manager for a healthcare client located remotely. This is a contract opportunity.

- Lead a multidisciplinary team in delivering holistic, person-centric care management for a diverse health plan population.
- Act as the primary contact for members, caregivers, and providers through various communication channels.
- Utilize the case management process to assess, develop, implement, monitor, and evaluate care plans across the care continuum.
- Coordinate with Medical and Behavioral Health Social Workers, Registered Dietitians, Pharmacists, and others.
- Assess members' health, psychosocial needs, and support systems, developing individualized care plans.
- Arrange necessary resources and coordinate care delivery among support systems, providers, and community agencies.
- Advocate for members and provide education on health literacy, self-management, medication plans, and nutrition.
- Monitor and evaluate care plan effectiveness, ensuring adherence to goals.
- Document interactions accurately to support member management.
- Prepare members for discharge or transfer, ensuring continuity of care.
- Secure durable medical equipment and transportation services and communicate details to members and relevant individuals.
- Must have at least 100 Mbps download speed, and 10 Mbps upload speed
- 3 years of clinical nursing experience in a clinical, acute/post-acute care, and community setting
- Nursing Diploma or Associate's Degree in Nursing
- 1 years case management experience in a managed care setting
- Certification in Chronic Care Professional (CCP)
- Case Management Certification (CCM)
- MI or Compact RN License
- Proficient in Microsoft programs (Excel, OneNote, Outlook, Teams, Word, etc.)
- Compact RN License
- Intermediate computer knowledge, typing capability and proficiency in Microsoft programs (Excel, OneNote, Outlook, Teams, Word, etc.)
- Bachelor's Degree Nursing
- Ability to think critically, be decisive, and problem solve a variety of topics that can impact a member’s outcomes.
- Proficient in motivational interviewing skills
- Demonstrated time management skills
- Organizational skills with the ability to manage multiple systems/tools, while simultaneously interacting with a member
- Excellent interpersonal and communication skills both written and verbal.
