Pioneer Guest Home Registered Nurse (RN)

What You Will Do

Wallowa Valley Center for Wellness is seeking a highly compassionate individual who loves making a difference in the lives of individuals with serious and persistent mental illness in our residential facilities.

In this vital role, you will be a part of a tight-knit team that works to ensure each and every person in our care lives their best life and reaches their goals. The RN is responsible for overseeing medication training and delegation of medication aides who provide residential support services to the residents. You will monitor the electronic documentation record and MAR, review physician's orders, perform routine and as needed nursing tasks and assessments, and create and maintain nursing care plans. You will ensure medical needs are addressed in a timely fashion and work with Pioneer Guest Home staff to ensure medical appointments are arranged when needed. You will work closely with the PGH administrator to ensure documentation needed for outside entities is accurate and complete.

Attendance/Reliability: WVCW expects staff to be regularly on time and at their designated place of work as planned. Staff are expected to plan ahead for vacation and anticipated appointments. When staff need unplanned time off they communicate this to all who need to know including clients.

Communication/Collaboration/Teamwork: WVCW expects staff to consistently offer clear and timely communication, collaborate as needed and work well within the team structure. This includes being assertive with information critical to the team and client care as well as being able to assimilate shared information promptly and accurately.

Taking Initiative/Problem Solving: WVCW expects staff to know their position, team, agency and client needs well and be able to advocate for needs and propose solutions. Staff must also recognize when input is needed from supervisor/ senior staff and will seek it appropriately.

Quality and Accuracy of Work/Completion of Tasks/Meeting Deadlines: WVCW expects completed work is of adequate quality and accuracy. Oversight is sought by staff as needed to ensure that standards for quality of care are met.

Your Responsibilities include but are not limited to the following:

  • Administer prescribed medications, following physician's orders for all medications and/or procedures, and documents in the electronic record and eMAR.
  • Monitor all aspects of medical and medication issues
  • Provide timely and accurate documentation Report any suspected violations of resident rights or abuse of a resident by another resident or staff member as a mandatory reporter
  • Be available to work with residents experiencing crisis situations
  • Be available to be on-call (paid at a daily rate based on evening, weekend, or holiday) on a rotating basis. While on-call, you will be accessible by phone and provide responsible support, documentation and follow up in a timely manner. This may include onsite visits.
  • Be knowledgeable about personnel policies, OARs pertaining to Residential Treatment Facilities and Community Nursing, and any other applicable rules or standards of pertinent regulatory agencies
  • Establish and maintain a positive working team environment with all facility staff and community partners, and treat everyone with dignity and respect
  • Participate fully in team meetings, training, and one-to-one supervision
  • Remain alert and provide direct line-of-sight monitoring of residents as needed
  • Other duties as assigned

The qualified Registered Nurse we are seeking is an individual with the following qualifications:

Required

  • Current and unencumbered Registered Nursing license in the State of Oregon
  • A valid driver's license
  • Ability to pass a criminal background check
  • Must be regularly available for work as scheduled

Preferred

  • Experience working in community nursing
  • Experience working in behavioral health

Knowledge, Skills, and Abilities

  • Ability to work with a high level of autonomy, use independent nursing judgment, and make nursing decisions
  • Ability to prioritize and manage time effectively
  • Ability to communicate effectively in both written and oral formats, including the ability to complete objective, accurate documentation in a timely, consistent manner
  • Ability to interact with individuals of various backgrounds, education levels, and socio-economic status
  • Must have basic computer skills including the ability to document in an electronic medical record, use electronic time tracking, and the ability to use Google Drive and email
  • Ability to maintain confidentiality, comprehend laws, regulations, and policies, and the ability to follow such laws, regulations, and policies and report when they are being violated

The Location

Pioneer Guest Home is a 16-bed residential treatment facility in Enterprise, Oregon for individuals with both physical and severe and persistent mental health issues who need some help with daily care, yet can still be independent at times. This will be your primary working location. This facility provides 24/7 oversight, room and board, all meals, medication management, case management, nursing care, skills training, and activity therapy.
