Base rate is $19-$22 with an additional $0.50 shift differential for evening and nights

- Observing Residents
- Reporting any health issues to the supervising nurse
- Taking care of a Resident's personal hygiene, including bed bath, shaving, etc.
- Setting up of meal trays, and documenting food/fluid intake
- Feeding Residents & serving nutritional supplements
- Making beds & keeping the Residents' space clean and tidy
- Transporting Residents within the Facility
- Turning bedridden residents to prevent bedsores
- Maintaining Confidentiality of all Resident & Facility data
- Must be able to work as a team member
- Successful completion of a CNA program
- Current CNA State Certification
- Must be in good standing with State Registry
Warren Center is an eighty-bed skilled nursing facility located at 42 Gurney Lane. Our facility provides a wide spectrum of nursing care to the residents of Warren County. We provide short-term rehabilitation services to people who cannot immediately return to their homes because of an accident or illness. We also provide long-term care for individuals who are unable to care for themselves. We look at each person from an individual perspective. We then utilize a team approach working with the residents and their families to develop a unique plan of care to meet their needs. We help children to reach their developmental goals through individualized therapy-based care and equip parents through monthly education workshops and support groups so they can be a confident advocate for their child. Our services include speech therapy, occupational and physical therapy, developmental services, nutrition as well as family education and support. With nowhere to turn for community-based services or help, families lacked support and their children had limited options in life. The Warren Center was created in 1968 and for decades, we have provided much-needed resources and care to children with disabilities and their families. As our organization experienced growth, we began adapting our programs to meet the changing needs of families.
