Description
Scope:

Responsible for the provision of comprehensive coordinated entry services within the Detroit Continuum of Care. Assist Coordinated Entry (CE) Manager in coordinating day-to-day operations and logistics at a CAM intake site in Detroit. The Coordinator is responsible for tracking daily shelter bed counts, approving placement of consumers into specific shelters, and ensuring the processes for assessment, shelter placement, and assignment to navigation occur for consumers in an efficient manner.
Essential Functions:
  • Coordinate day-to-day operations and logistics at a CAM intake site which includes, but is not limited to: daily shelter bed inventory, appropriate referrals to shelter, coordination with street outreach providers, assigning workload to Specialist staff
  • Work collaboratively with other Coordinators assigned to a CAM intake sites
  • Attend and actively participate in workgroups, committees and case conferences related to emergency shelter and diversion
  • Address programmatic needs and provide staff support as needed to ensure optimal provision of services for households experiencing homelessness
  • Ensure overall documentation and data entry requirements are met at assigned CAM intake site
  • Maintain knowledge of Coordinated Entry System performance measures; including those specific to Housing & Urban Development (HUD) and Detroit Continuum of Care internal measures
  • Responsible for adherence to program, Continuum of Care (CoC) and State of Michigan HMIS policies, practices and procedures
  • With guidance from supervisor, create, troubleshoot, and revise as needed processes and procedures to ensure timely and accurate data entry from staff.
  • Conduct client phone screenings and intakes as needed; collecting required documentation, assessing acuity and prioritizing need for services
  • Input data into the Michigan State Housing Development Authority's (MSHDA) Online Waiting List Pre-Application database for Housing Choice Vouchers (HCV). Track data and clients progress in the recertification process.
  • Maintain a positive attitude towards assigned work, clients, and staff, and a willingness to work with others
  • Perform work in a professional manner that upholds Wayne Metro's policies and procedures
  • Other duties as assigned
Qualifications
Minimum Qualifications:
  • Bachelor's degree; OR a combination of education and experience that is similarly equivalent.
  • A minimum of four years of experience in a related field.
  • Working knowledge of Detroit Continuum of Care and/or homeless services is a must, or transferable skills required.
  • Ability to collect and evaluate data regarding service needs, present services, and service gaps; and through such an evaluation, develop means through which to address those service gaps. Experience with the Michigan Homeless Management Information System (HMIS) preferred but not required.
  • Possess strong organizational, collaborative, and interpersonal skills to ensure that supportive services are delivered to eligible clients through engagement, education, and systems development.
  • Excellent writing and oral communication skills
  • Ability to organize, plan, prioritize, meet deadlines, and work under pressure
  • Flexible and committed to working as a team toward pursuit of common goals.
  • Ability to use sound judgment and initiative to analyze and resolve problems.
  • Effective listening skills and professional communication skills, both orally and in written form.
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of individuals in a diverse community.

Minimum Competencies:
  • Working knowledge of basic internet applications and programs including Google technology and Microsoft Office Word and Excel. The ability to learn and utilize statewide data collection processes necessary to programs for this position.
  • Good interpersonal and professional skills including but not limited to professional appearance, tact, punctuality, and dependability.
  • Ability to provide services for those persons facing economic, social, health, age, and educational barriers.
  • Ability to work independently or in a team environment
  • Perform work in a manner that upholds Wayne Metro policies and procedures.
Travel Requirements:
  • Ability to travel to various sites throughout Wayne County.
Licensing and Other Requirements:
  • Valid identification and reliable transportation.
  • Although the Covid-19 vaccination is no longer mandated, it is encouraged. Wayne Metro will always prioritize the safety of our staff and clients.
Work Hours:

Available work schedules range in timeframes from Monday - Saturday, with specific hours to be determined by department needs.
Physical Requirements and Work Environment

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. This job operates within a homeless service provider environment with a moderate noise level. Services may be provided in an office or community setting. Occasional driving between intake sites may be required.. This is largely an active role and the employee must occasionally lift and/or move up to 25 pounds. While performing the duties of this job, the employee is regularly required to walk, sit, and frequently use hands and to talk or hear.
Specific vision abilities required by this job include close vision, distance vision, peripheral vision, color vision, and the ability to adjust focus.
