Waystone HHS is Hiring a DAY HABILITATION COORDINATOR, 40 Hours, Mon-Fri 8a-4p Near Haverhill, MA

PRIMARY PURPOSE: The Day Habilitation coordinator will oversee the classroom to support the daily programming and to assure that all aspects of the client’s services provide a purposeful and meaningful day that aligns with the agency values and expectations.
LOCATION: Parkridge, Haverhill, MA
SCHEDULE: 40 Hours, Monday - Friday, 8:00 am - 4:00 pm
Essential Functions
  • Hire, supervise, evaluate, and train developmental specialists on required goal tracking for the individual DHSP goals & data collection.
  • Maintain the required documents for the assigned caseload, including developing and training on the day habilitation support plans. This includes ensuring that all required documentation is accounted for to meet timelines.
  • Assists with in-take and orientation process for new members and helps to ensure that all service assessments are completed within the first 45-day period. Work with the clinicians to ensures that individuals are evaluated to determine service needs.
  • Ensure implementation of members’ DHSP objectives and corresponding data collection and document progress and needs of members in monthly and quarterly reports.
  • Oversee the training on the implementation of support plans to maximize skill acquisition and goal attainment. Monitor quality of performance and outputs of persons served- adjusting techniques when necessary. Perform ongoing review of all plans to ensure updates are made when needed.
  • Know, understand, and train on the profiles and histories of person served and active indications that care and assistance follows the prescribed needs. Understand and implement crisis plans, behavior treatment plans, and medical protocols correctly and consistently.
  • Complete all documentation required by regulatory agencies, focusing on providing complete, relevant, and accurate information. Ensure all paperwork and/or data entry is completed within documented deadlines.
  • Provide room coverage as needed to ensure proper day habilitation ratios are adhered to, including providing personal care and assistance to individuals consistent with their medical and physical needs.
  • Work in conjunction with the Director to ensure all curriculum and activities are aligned within an identified service need area outlined by mass health regulations and standards.
Other Responsibilities
  • Provide ongoing evaluation of the wants and needs of the person served. Ensure feedback is collected for all aspects of programming such as activities, support plans and community. Utilize feedback to aid in future decision making thus enabling a detailed person-centered plan.
  • Provide opportunities for self-advocacy and input from persons served into their plan of care. Proactively identify concerns and solutions regarding service delivery and work in conjunction with Director, classroom supervisor, Clinicians, and team members to ensure consistent, effective, and person-driven supports for persons served. *
  • Perform all duties in a manner which demonstrates an awareness and respect of everyone’s legal, civic, and human rights. Maintain and enforce the confidentiality of any information relating to individuals served.
  • Establish or maintain a professional relationship with person served and all stakeholders involved in their plan of care (i.e., family members, guardians, providers, funding sources, etc.). Provide ongoing communication and updates, as needed.
  • Maintain the member’s record for assigned caseload. Ensure all required documentation is present and updated to meet expected timelines. Assist in preparation for regulatory and accreditation (OQE, CARF and Mass Health) visits through the completion of audits and completion of required trainings and documents with clients and with the department team.
  • Work collaboratively with team members. Attend all assigned meetings as an active participant, including the monthly interdisciplinary team meetings. Ensure coverage and support for other service managers or team members as needed.
  • Communicate with Director, classroom supervisor, and program health care supervisor any medical issues pertinent to the health and safety of the people served.
  • Assist in maintaining a clean and safe environment by reporting issues to the proper personnel.
  • Provide transportation as requested by supervisor.
  • Complete all mandatory trainings within 90 days of hire date. Attend and/or complete all ongoing trainings, as indicated by the Waystone Health and Human Services.
  • Other duties as assigned by supervisor.
Requirements
  • High School diploma or GED.
  • Computer literacy and exceptional organizational, writing, time management, presentation and people skills are necessary.
  • Valid driver’s license with minimum of 1 year driving experience.
  • Commitment to the inclusion of people with developmental disabilities in their communities as valued employees and citizens is required.
  • Experience with case managing and case reviews in a related health care setting preferred.
  • Minimum of one year of experience in the delivery of services to individuals with developmental disabilities preferred.
  • Regular and predictable attendance
