We are seeking a dedicated Social Worker to join our team. The ideal candidate will provide support and advocacy to individuals and families in need, focusing on various aspects of their well-being. This role involves working closely with registrants to address their social, emotional, and financial challenges.

Duties
· Assist in the implementation of the social work plan of care.

· Prepare timely, periodic social services progress notes, documenting services provided, registrant status and progress made.

· Act as an advocate for the registrants and their caregivers.

· Serve as a liaison between registrants/caregivers and various governmental/regulating agencies.

· Assist with registrants’ personal affairs including burial arrangements, when necessary.

· Assist in completing referral information, including initial intake form, logging referrals and follow-up phone calls to referral sources and MDs.

· Assist in admission and discharge planning for registrants.

· Participate in interdisciplinary team meetings, care plan meetings, and family meetings, as indicated.

· Arrange health education and/or support meetings for registrants and caregivers.

· Assist registrant in securing services determined to be needed through referral to agencies or persons qualified to provide those services.

· Assist registrants with any forms or applications for needed community based services, aide or assistance they may benefit from.

· Assist in providing crisis intervention in the event a participant’s health and well being are jeopardized or at risk of being jeopardized

Qualifications

1) Bachelor’s degree in Social Work or Human Services related area, with pertinence experience, is preferred.

Or,

2) Significant experience or educational training in health or social services and work experience involving case management for community-based population.

Pay: $20.00 - $25.00 per hour
