WELLBE INTRODUCTION

The WellBe care model is a provider led Advanced Geriatric Care Program focused on the quality of care of the frail, poly-chronic, and elderly Medicare Advantage patients. This population is typically underserved and very challenged with access to overall health care. To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. WellBes Care Model is to provide our members the entire continuum of care from chronic to urgent care in the home, hospital, skilled nursing facility, assisted living, palliative care, and to end of life care. WellBe's physician/advanced practicing clinician led geriatric care teams partner with the patients primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum.

GENERAL SUMMARY

WellBe Senior Medical is building a team of passionate, resourceful and dedicated team members to focus on market-based engagement. As the supervisor of the Community Engagement Specialists, you will be responsible building and leading a team to enable the development and execution of innovative ideas to market the value of WellBe Senior Medical to attributed Medicare patients. Through in-person, telephonic outreach and community events, your team will build trust-based relationships with attributed patients leading to their engagement with WellBe Senior Medical. The team is also accountable for promoting WellBe Senior Medical meeting patients where they are, i.e physician offices, hospitals, sub-acute facilities, and long-term care settings, homes.

RESPONSIBILITIES

• Build and manage a high-performing team, fostering an environment of continuous learning and development.
• Provide mentorship and guidance to team members, enhancing their skills and career growth.
• Support the effort to identify innovative strategies to enroll engage patients with WellBe.
• Daily reporting, documentation and detailed pipeline management to prioritize telephonic and field-based outreach to patients and the performance of Community Engagement Specialists
• Outbound telephonic outreach to engage patients.
• Build awareness of the benefits of WellBe with attributed patients through multiple grassroots activities including health fairs, presentations at senior buildings, canvassing and activities in communities with critical mass of attributed patients.
• Uses critical thinking skills to create outreach strategies that are designed to maximize patient awareness and engagement with WellBe.
• Successfully meet and/or exceed engagement monthly targets.
• Address patient concerns and schedule for an initial visit from a provider
• Follow up with patients to ensure they keep their scheduled initial visit
• Provide exceptional, compassionate customer service

Educational Requirements :

• High School Diploma/GED required, Bachelors Degree preferred

Required Skills and Abilities:

• 4 years of related experience in a sales, direct marketing, community relations or customer service capacity. Prior experience in healthcare preferred.
• 2 years supervisory experience
• Exceptional communication skills and emotional intelligence, with the desire to help patients live a meaningful life.
• Comfort with ambiguity and ability to proactively structure work and problem solve independently
• Proven success with time management and territory organizational skills
• Achievement mindset: ability to consistently hit goals and motivated by performance-based compensation
• Proficient in the use of Microsoft office Word, Excel, and PowerPoint with strong written and public communication skills
• Access to a vehicle that you can use every day in job functions. Mileage is
• Familiarity with customer relationship management (CRM) systems preferred

• Hybrid role with expectations of being in the office for meetings and events and in the field to conduct the responsibilities of the role.
