WellLife Network is Hiring a YCM Program Director - YCM031 - Mon - Fri 9AM-5PM Flexible Near Coram, NY

Position Summary: Reports to the Division Deputy Director of the Child & Family Services department. The Youth Care Management (YCM) Program Director oversees all aspects of the Youth Care Management program. The Program Director is responsible for establishing and implementing policies and procedures, maintaining operations and clinical functions of the program. Supervises the YCM Program Coordinator(s) and Youth Care Coordinators, as necessary.
Essential AccountabilitiesADMINISTRATIVE
  • Create and nurture effective communication within the organization.
  • Build a strong team through open communication and by collaborating on decision-making responsibilities.
  • Initiate and set goals for the YCM program based on the organization’s strategic objectives.
  • Provide leadership with systemic and programmatic design.
  • Responsible for the recruitment, hiring and initial training process of staff.
  • Ensure ongoing training and development of staff.
  • Assist in engaging children and families using a family-driven, youth-guided, strengths-based, culturally competent, trauma informed approach.
  • Provide ongoing supervision of staff.
  • Obtain and maintain CANS-NY Assessment Certification (Supervisory staff must obtain an 80% or higher on certification test).
  • Facilitate team meetings.
  • Develop, implement, and maintain program policies, procedures and protocols in accordance with federal, state, oversight agency and organizational regulations.
  • Oversight of program documentation. Periodically conduct internal audits of client charts.
  • Perform program evaluation and Quality Assurance.
  • Ensure programmatic compliance with agency incident reporting guidelines.
  • Monitor and report on program revenue.
  • Complete monthly and annual reports, as needed.
  • Approve program purchases and maintain petty cash funds.
  • Develop, implement and monitor Performance Improvement Plans.
  • Ensure completion of consumer evaluation process and summary reports.
OPERATIONAL
  • Review of all referrals and eligibility determinations. Assure maintenance of eligibility documentation.
  • Provide ongoing supervision of the care of clients.
  • Oversee discharge process. Review appropriateness of client discharge and transitional resources. Supervise the implementation of the discharge plan.
  • On-call as needed for programmatic issues and challenges.
  • Maintain relationship and facilitate communication between the program and Health Homes.
Other Responsibilities
  • Participation in the development and implementation of division trainings.
  • Attend workshops, conferences, and trainings as requested.
  • Responsible for maintaining (or having access to) a personal vehicle throughout the course of the work day.
  • Maintenance of an automobile insurance policy.
  • Other duties as assigned.
The preferred candidate is a Masters-level Human Services professional with experience working with youth and their families. The preferred candidate will have experience with the Health Home Serving Children program, Health Home systems, care coordination, as well as strong and effective leadership skills, communication skills, and program development skills. Minimum qualifications for the position is a Bachelor of Arts or Science Degree with two years of relevant experience. Must obtain and maintain CANS-NY Certification. Must maintain NYS Driver's License in accordance with WLN's driving policy
