Description

POSITION TITLE:  BILINGUAL PARENT NAVIGATOR
PROGRAM:  Community Support for Families 

GENERAL STATEMENT OF RESPONSIBILITIES:

Community Support for Families Parent Navigator(s) provides direct support and service coordination to families in Region 5 CSF. CSF Parent Navigators utilize strength-based, family-driven, youth-guided, culturally responsive principles to develop individualized mentoring and engagement with families. The purpose of Community Support for Families is to ensure that services are delivered in a coordinated and therapeutic manner, and are consistent with the core values and guiding principles of the Strengthening Families and WRAP-CT principles. Parent Navigators work with the CSF worker to link families to formal and informal supports in the community.

SUPERVISION:  Supervised by: Team Leader, Community Support for Families

EXAMPLES OF RESPONSIBILITIES

  • Provides empowerment and advocacy for families, focusing on assisting families in developing effective partnerships with schools, providers and others to best meet their children and family’s needs.
  • Conducting assessment and data entry to track client's progress through EHR
  • Provides peer support and role modeling to teach families how to identify needed resources and supports in their communities.
  • Supports direct case management services and supports the development of case management plans specific to each assigned client and family, including interventions necessary to increase the probability of treatment completion, and to improve life functioning.
  • Maintains records of direct services for reporting and tracking purposes and completes all documentation for client record in accordance with documentation timelines.
  • Attends team meetings and staff meetings to assist in the development of case management needs and service plan reviews.
  • Provides services in the community outside of the office (e.g., homes, schools, and other community locations) as defined by the program.
  • Develops and maintains positive relationships with community partners, providers, and other stakeholders.
  • Supports implementation of safety planning with appropriate supervision and as defined by program.
  • Other duties as assigned by the Supervisor

Qualifications


QUALIFICATIONS:

EDUCATION AND EXPERIENCE:

Community Support for Families Parent Navigators should possess a college degree (AA/AS or BA/BS) in a human service field (e.g., social work, marriage and family therapy, counseling, or psychology, etc.). Persons with a degree in an unrelated field or without a college degree may be considered if they have at least 3 years’ experience working with families and knowledgeable about community resources and services preferred.

CSF Worker must have the skills necessary to meet the cultural and linguistic needs of the families to be served by the program. Bi-lingual (Spanish-English) is required.

LICENSURE/CERTIFICATION/REGISTRATION:

Valid driver’s license, reliable transportation and valid auto insurance is required. 
This role may require travel throughout the Region 5 area which includes (Waterbury, Torrington, and Danbury)
