Job Summary:
We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, collaborating with healthcare professionals, and ensuring optimal outcomes for patients.
Job details:
Location: 3950 Austell Rd, Austell, GA 30106
Duration: 13 weeks with possible extension
Shift: 12 hours day shift, any 3 days in a week with every other weekend
Requirements:
- Minimum 1-2 years of Acute care experience
- Active GA RN License/Compact License
- BLS/CPR certification
Responsibilities:
- Manage patient cases by assessing needs, developing care plans, and coordinating services.
- Utilize nursing expertise to provide quality patient care and ensure patient safety.
- Collaborate with healthcare team members to optimize patient outcomes.
- Conduct patient assessments and implement appropriate interventions.
- Ensure compliance with HIPAA regulations and medical terminology standards.
- Coordinate discharge planning and post-acute care services for patients.
