Nursing Case Manager Health Services DepartmentDuties:-Responsible for identification, organizing and coordinating resources and services to meet healthcare needs of high-risk students across the district-Work in collaboration with students, family/caregiver(s), district staff, clinic/hospital/specialty providers and community resources to create a team approachQualifications, Education and Experience:-RN licensure in the Commonwealth of Massachusetts-BSN required-DESE licensed as a School Nurse or eligible for licensure-Current Healthcare Provider CPR/AED certification required-Must have reliable form of transportation -One (1) to three (3) years’ experience in case management, pediatric home care, and/or school nursing required. Strong demonstrated knowledge of community health services required.-Excellent interpersonal, organizational and communication skills and ability to collaborate effectively Salary: Per MNA Contract
