Job Duties***This position is eligible for a sign on bonus of up to $5,000. Must be new to state service in order to be eligible***
Western State Hospital is seeking a full time Clinical Social Work Supervisor/Counselor II to provide supervision and performance management of social work staff on CMS certified and JC Accredited treatment units and to ensure that social work services are in concert with the mission of Western State Hospital (WSH); to develop and implement person-centered treatment and discharge plans in collaboration with consumers and other members of the interdisciplinary treatment team and serve as link between community agencies and others that support consumer's recovery; supervision of and responsibility for the delivery of social work services in 3 Forensic Service Units, independently monitor staff compliance with Joint Commission Standards, WSH and Social Work Dept. policies an procedures; and requirements of the performance planning and evaluation systems are met, and evaluations are completed by established deadlines with proper documentation.
The Clinical Social Work Supervisor/Counselor II will also provide training, monitoring of staff work performance and provide coaching/mentoring to supervisees to ensure delivery of quality services for each patient; assist Clinical Social Work Director in the development of SW Department standards of practice and development and implementation of policies and procedures; provide SW assessments and identifies discharge needs; provide PSR interventions and participate in development and implementation of individualized treatment plans; and provides link between WSH treatment team an consumers' families and other persons that support recovery.

Minimum Qualifications
  • MSW or MSSW required (graduation from a CSWE accredited undergraduate or graduate social work program)
  • professional work experience with adults with serious mental illness
  • experience in discharge planning
  • comprehensive knowledge of social work theory and practice from an accredited social work degree program
  • knowledge of functioning of hospital and community mental health systems
  • ability to form therapeutic relationships with consumers and their supports, and to form effective partnerships with community service providers
  • demonstrated skills in interviewing, assessment, problem solving, professional collaboration, and the ability to coordinate community and agency resources
  • knowledge of serious and persistent mental illnesses, forensic issues, person centered treatment, and discharge planning, as well as community resources available to those with mental illnesses
  • knowledge of supervisory principles, performance planning, and evaluation
  • ability to work effectively in a setting in which interdisciplinary collaboration is highly valued
