Westhills Village is recruiting for a full-time, day shift, Social Services Coordinator to join our health care team. The Social Services Coordinator provides social work services consisting of assessment, counseling, admissions, and discharge planning. This position would be ideal for someone with a LPN/RN License.

Some Qualifications:

• Bachelor’s degree in social work or similar field or LPN/RN License
• Two years’ experience in a social worker role preferred, preferably in a health care setting.
• Previous case management experience
• Excellent oral and written communication skills, including ability to compose correspondence.
• Detail oriented with strong organizational skills.
