Westmass Eldercare is Hiring a Bilingual Care Coordinator Near Holyoke, MA

WMEC offers a hybrid schedule to all employees after completion of 6 month training period. Our hybrid schedule allows employees to work up to 2 days remote and 3 days in the office. Our offices are located in Holyoke, MA.

The Care Coordinator ensures high quality services are provided to children and adults ages 3-64 who have medical, behavioral health, and/or substance abuse issues. Working as part of an interdisciplinary team, the Care Coordinator provides care coordination. The Care Coordinator may also provide education to enrollees regarding the benefits and purpose of the CP program, how the assigned CP will work with the enrollee, and the importance of engagement and participation of the member within the CP program.

ESSENTIAL JOB FUNCTIONS*

  • Provides telephonic and in-person care coordinator based on sound clinical theories and evidence-based practices.
  • Provides support for transitions of care including participating in discharge planning, coordinating clinical services and other supports for the Enrollee, as needed.
  • Monitors the effectiveness of care plans and coordinates with enrollee and care team to make changes as needed in order to promote positive health outcomes and responsible use of resources.
  • Demonstrates willingness and ability to work with challenging and diverse enrollee populations.
  • Demonstrates ability to collaborate with a team of interdisciplinary professionals.
  • Demonstrates professional skills including conveying and responding to written and oral information effectively.
  • Provides ongoing Care Coordination support to the Enrollee in coordination with the Enrollee’s PCP and other providers.
  • Provides health and wellness coaching to all Enrollees.
  • Develops and maintains collaborative relationships with state agencies and other community-based organizations, and have knowledge of the services and specialties offered.
  • Demonstrates cultural and linguistic competence and principles of health equity in working with enrollee’s with a variety of different cultural and socioeconomic backgrounds.
  • Demonstrates working knowledge of community resources and how to access them.
  • Completes paperwork and any additional specific forms and documents that are required within required timelines.
  • Meets program goals and timelines as defined by Program Director, or designee
  • Ensure quality communications to enrollees and members of the LTSS CP team
  • Provides assistance and back up to LTSS CP team members as directed by Staff Supervisor and/or Program Director.
  • Knowledgeable with program policies and procedures

OTHER DUTIES AND RESPONSIBILITIES

  • Reviews and responds to email within 1 business day
  • Reviews and responds to phone messages within 1 business day of receipt
  • Demonstrates an ability to effectively manage competing priorities and time management.
  • Has consistent and punctual attendance to required meetings as required by supervisor or designee
  • Completes all other responsibilities as required by the supervisor and program.
  • Completes other routine assessments as required by the program.

  • Care Management- Embraces the care management philosophy by assisting clients with medical, behavioral health, and/or substance abuse concerns with navigating the healthcare system to improve health outcomes as well as addressing social determinants of health needs

PREPARATION, KNOWLEDGE, SKILLS & ABILITIES

  • Broad knowledge of physical and mental illnesses, developmental disabilities, SUD, and the impact of social determinants on overall health preferred
  • Satisfactory background check that includes CORI, SORI, SAMs GSA, Mass Health Exclusion List and OIG check
  • Valid driver’s license required.
  • Vehicle available during all working hours. Vehicle should be Registered and inspected in accordance w 540 CMR 4.00.
  • Familiarity with Internet, email systems, electronic health records (EHRs), and databases.
  • Educational background needed:
  • High School Diploma or GED required along with at least 4 years of relevant professional experience; or an associate’s degree and at least two-years professional experience in the field; or a Bachelor’s Degree in social work, human services, nursing, psychology, sociology, or a related field and at least one-year professional experience in the field
  • Ability to work in an office setting during normal business hours

Pay: From $21.00 per hour
