Job Description

The Dozoretz Hospice House of Hampton Roads is hiring RNs for various shifts.

Day Shift: 7am-7pm

Night Shift 7pm- 7am

I. QUALIFICATIONS

  • I Graduated from an accredited school of nursing.
  • Currently licensed as a registered nurse to practice in Virginia.
  • Minimum 2 years of experience as a registered nurse in hospice/home health
  • Active patient contact within the past three years, preferred.
  • Ability to work independently, make accurate, and at times, quick judgments.
  • Ability to supervise others appropriately.
  • Ability to respond appropriately to crises.
  • Demonstrated ability in or application of organizational/communication skills.
  • Ability to deal effectively with high levels of stress.
  • Prior experience using electronic health records, preparing/entering clinical notes.
  • Strong computer skills, including Microsoft and Google applications.
  • Ability to multitask and meet deadlines.


II. RESPONSIBILITIES

  • Ensures compliance with federal, state, and local regulations, accreditation standards, and hospice policies in all hospice program patient care aspects while optimizing comfort and dignity.
  • Provides hospice nursing services, treatments, and preventive procedures, ensuring safe and therapeutically effective services to patients.
  • Regularly assesses and reassesses the nursing needs of the hospice patient, ensuring all nursing needs are met, as identified in the patient’s initial assessment, comprehensive assessment, and updated assessments.
  • Actively participates in quality improvement reviews and evaluations of nursing services provided, and contributes to IDG meetings as requested.
  • Review records for compliance with regulations.
  • Ensures the patient is being nourished as appropriate working with the Hospice House team.
  • Implements the plan of care and initiates nursing procedures appropriate for the patient’s hospice care and safety.
  • Obtains physician’s orders when necessary.
  • Observes signs and symptoms and coordinates and communicates reports to the physician and IDG members, any unexpected changes in the patient’s physical or emotional condition.
  • Coordinates care with internal and external interdisciplinary team members, including social workers, chaplains, and home health aides.
  • Teaches, supervises, and counsels the hospice patient and family members regarding care needs, including pain management.
  • Supervises, orients, and trains other nursing service personnel as requested.
  • Prepares patient-specific instruction for hospice aides/certified nursing assistants.
  • Develops and re-evaluates the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care.
  • Provides services requiring substantial and specialized nursing skill; performs specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctor’s orders.
  • Evaluates the outcome of services.
  • Maintains accurate and complete medical record documentation.
  • Supervises licensed practical nurses, hospice aides and/or certified nursing assistants.
  • Observes confidentiality and safeguards all patient-related information in compliance with HIPAA regulations.
  • Always communicates to the supervisor if unable to meet a patient’s need or perform a

procedure.

  • Participates in on-call system and is responsible for providing on-call coverage when

unavailable for assigned duties.

  • Maintains skills and knowledge, keeping all required certifications up to date.
  • Participates in quality improvement initiatives and continuing education to maintain up-to-date knowledge and skills.
  • Organizes work schedule and utilizes time management to be able to attend all required

meetings and team conferences.

  • Complies with current infection control policies and protocols.
  • Immediately reports any accident, incident, lost articles, or unusual occurrence to the IPU Director/Clinical Coordinator.
  • Adheres to the HHHR code of ethical behavior and models our core values of respect, passion, and professionalism at all times
  • Performs all other duties as assigned.


III. JOB CONDITIONS

  • Physical Requirements:
    • Requires considerable physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes and maximal assist in lifting and/or transferring of a 100-pound patient.
    • Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position
    • Must provide evidence of annual TB test and other state-required tests or examinations.
  • Mental Requirements:
    • Must be able to work independently, make judgments based on assessments and data available and act accordingly.
    • Must be flexible, innovative and possess good interpersonal skills.
    • Must be able to cope with mental and emotional stress and demonstrate emotional stability.
  • Working Conditions:
    • Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids and excrements, adverse environmental conditions and hazardous materials.


IV. ORIENTATION AND TRAINING

Dozoretz Hospice House of Hampton Roads will provide orientation and training, as necessary, to assist the Registered Nurse in performing the responsibilities listed above. Training may be held in-house, or outside seminars. Evidence of orientation and training will become part of the personnel file.


V. ACCOUNTABILITY

  • Reports directly to the Hospice IPU Clinical Coordinator
  • Works cooperatively with all members of the Interdisciplinary Team

Company Description

The Dozoretz Hospice Hospice House of Hampton Roads is a 12 bed Inpatient Hospice facility opening in the Spring of 2024. Beth Sholom and Westminster Canterbury on Chesapeake Bay have partnered to operate and manage the house.
